How dose calculations actually work
Most veterinary medications are dosed as "milligrams of drug per kilogram of pet body weight." So a drug dosed at 5 mg/kg given to a 10 kg dog needs 50 mg of active ingredient. If your pill or liquid contains 25 mg per unit, you'd give 2 units.
That sounds simple, and for many drugs it is. But three things complicate it in practice:
- Concentration math. Liquid medications have a concentration (mg per mL). Pills have strength (mg per pill). You need to convert between the dose in milligrams and the actual amount you administer.
- Species differences. Cats and dogs metabolize many drugs very differently. A drug safe for dogs at a given dose can be fatal to cats at the same dose. Tylenol (acetaminophen) and ibuprofen are textbook examples — common in human medicine, deadly in cats and very risky in dogs.
- Individual variation. A standard dose works for most animals but causes problems in some. Liver and kidney function affect how drugs are processed; breed differences affect certain pathways (Collies and some herding breeds carry an MDR1 gene mutation that makes them dangerously sensitive to common drugs like ivermectin).

That table includes things you should NOT give. The most common pet poisonings from owner well-intentioned dosing are: acetaminophen given to a cat (fatal), ibuprofen given to a dog (kidney failure), and hydrogen peroxide given to a cat (severe esophageal damage).
The species difference problem
Cats can't process many drugs that are safe for humans and dogs. The reason is a specific liver enzyme deficiency — cats lack adequate UGT1A6, which is responsible for "glucuronidation" of certain compounds. The result is that drugs that humans clear in hours can build to toxic levels in cats within a single dose.

Why your vet may seem expensive
Veterinary medicine is the same science as human medicine but without insurance pools spreading the costs. A CT scan in human medicine is $1,500 but the insured patient pays $200. A CT scan in veterinary medicine is also $1,500 — the owner pays $1,500. Same equipment, same expertise, full cost.
This creates the common surprise of "the vet bill was $3,000." It's not that the vet is gouging. It's that you're paying directly for a service insurance normally subsidizes in human medicine. Common dosing mistakes
- Confusing pounds and kilograms. 1 kg = 2.2 lb. A 10 kg dog is 22 lb. Use the right unit for the dose. Dosing by lb when the protocol is mg/kg gives 2.2× the intended dose — often dangerous.
- Misreading milligrams vs milliliters. A 25 mg/mL solution given as "25 mL" instead of "25 mg" delivers 25× the dose. Read labels carefully.
- Using human medication concentrations. Baby aspirin is 81 mg per pill; adult aspirin is 325 mg. Confusing these has caused overdoses.
- Splitting unsplittable pills. Some pills (extended-release, enteric-coated) lose their formulation when split. Get the right strength pill rather than approximating.
- Continuing dosing past the prescribed duration. Antibiotics need full courses; pain medications often need tapering. "He seems better" isn't a reason to stop early or extend.
- Sharing medications between pets. A drug prescribed for one pet may be wrong for another, even at the right dose.

Some pet medications aren't available in pill sizes appropriate for small animals, or in flavors pets will accept. Compounding pharmacies (specialized pharmacies that custom-formulate medications) solve this by creating chicken-flavored liquid versions of pills, or tiny doses sized for a 4-pound cat.
